Service Description: <div style='text-align:Left;'><div><div><p style='margin:0 0 11 0;'><span><span>This map document and geodatabase includes the overlay features used on the CDC Atlas of Heart Disease and Stroke.</span></span></p><p style='margin:0 0 11 0;'><span><span>Hospital and health center locations are accessed from the below data sources. </span></span></p><ul><li><p style='margin:0 0 11 0;'><span><span>Federally Qualified Health Centers (FQHCs): </span></span><a href='https://bphc.hrsa.gov/about-health-center-program' style='text-decoration:underline;'><span style='text-decoration:underline;'><span>Health Center and Look-Alike Sites</span></span></a><span><span> are downloaded from the Health Resources and Services Administration </span></span><a href='https://data.hrsa.gov/data/download' style='text-decoration:underline;'><span style='text-decoration:underline;'><span>Data Downloads</span></span></a><span><span>. FQHCs are included if they meet the below criteria:</span></span></p><ul><li><p style='margin:0 0 11 0;'><span><span>Setting is a “Hospital” or “All other clinic types”</span></span></p></li><li><p style='margin:0 0 11 0;'><span><span>Status is “Active”</span></span></p></li><li><p style='margin:0 0 11 0;'><span><span>Location is “Permanent”</span></span></p></li><li><p style='margin:0 0 11 0;'><span><span>Type is “Service Delivery Site” or “Administrative/Service Delivery Site”</span></span></p></li></ul></li><li><p style='margin:0 0 11 0;'><span><span>Hospitals with Listed Services: Data are accessed from the </span></span><a href='https://www.ahadata.com/aha-annual-survey-database' style='text-decoration:underline;'><span style='text-decoration:underline;'><span>American Hospital Association Annual Survey Database</span></span></a><span><span> through a data use agreement.
