This map document and geodatabase includes the overlay features used on the CDC Atlas of Heart Disease and Stroke.<\/span><\/span><\/p> Hospital and health center locations are accessed from the below data sources. <\/span><\/span><\/p> Federally Qualified Health Centers (FQHCs): <\/span><\/span>Health Center and Look-Alike Sites<\/span><\/span><\/a> are downloaded from the Health Resources and Services Administration <\/span><\/span>Data Downloads<\/span><\/span><\/a>. FQHCs are included if they meet the below criteria:<\/span><\/span><\/p> Setting is a \u201cHospital\u201d or \u201cAll other clinic types\u201d<\/span><\/span><\/p><\/li> Status is \u201cActive\u201d<\/span><\/span><\/p><\/li> Location is \u201cPermanent\u201d<\/span><\/span><\/p><\/li> Type is \u201cService Delivery Site\u201d or \u201cAdministrative/Service Delivery Site\u201d<\/span><\/span><\/p><\/li><\/ul><\/li> Hospitals with Listed Services: Data are accessed from the <\/span><\/span>American Hospital Association Annual Survey Database<\/span><\/span><\/a> through a data use agreement. The Annual Survey collects data from more than 6,200 hospitals and health care systems. <\/span><\/span><\/p><\/li> Stroke Centers by Certification: Stroke center designations are collected from several different certification organizations. <\/span><\/span><\/p>
